2022 License Renewal
EVEN IF YOU HAVE ALREADY PAID RENEWAL FEE ONLINE, LICENSES WILL NOT BE RENEWED UNTIL THE FORMS INCLUDED IN THIS PACKET ARE COMPLETED AND RETURNED WITH PROOF OF 10 CEUS AND CALIBRATION CERTIFICATES
Kansas Hearing Instrument Fitting and Dispensing Renewal Form
• Fill out all personal and business information
• Fill in CEU hours obtained from KHS, KSHA, and/or other organization/manufacturer
• Answer all questions on page 2 of renewal form, sign and date
• Owners must also fill out the employee and branch location form
Continuing Education Units
• Proof of 10 hours of CEUs within the timeframe of 7/1/2021 – 6/30/2022 must accompany renewal forms
• Maximum of 3 hours may be ancillary (non-hearing aid related)
• Due to the COVID-19 Outbreak all 10 CEUs may be obtained online - for 2022 renewal period ONLY
Calibrations
• Proof of audiometer calibrations must accompany renewal forms
Payments
• Online payment is preferred! Payment by check may delay processing of renewal.
• To pay online, visit www.kbhae.com and click on “Online Payment”
• Please, include your receipt from online payment with renewal packet
• The renewal fee is $100 for active status or $25 to renew as “inactive”
Submitting Renewal Forms
• Emailing completed renewal forms to mark.shaver@ks.gov is preferred! Submitting forms by mail to P.O. Box 552, Rose Hill, KS 67133 will delay processing of renewal.
• All renewals received by May 14th will be mailed by the state printing office the first week of September
• Renewals received after May 17th will be processed and mailed by the board as time permits
• Renewals must be postmarked or emailed by June 30th to avoid a $200 late fee
